• بچے کے کمپیوٹرائزڈ پیدائش سرٹیفکیٹ کے ہمراہ والدین میں سے کوئی ایک یا عدالت سے متعین سرپرست (گارڈین) تشریف لائیں
• دونوں والدین موجود ہوں تو ایک درخواست دہندہ اور دوسرا تصدیق کنندہ ہو گا
• اگر والدین میں سے کوئی ایک موجود ہوں تو درخواست فارم کی تصدیق گزیٹڈ آفیسر یا عوامی نمائندے (ایم این اے، ایم پی اے یا بلدیاتی اداروں کے عہدیدار اور ارکان) سے کرانا ہو گی۔
• شناختی کارڈ کے لئے بچے کی موجودگی لازم ہے۔ بچے کی تصویر اور 10 سال سے زائد عمر بچوں کے فنگرپرنٹس بھی لئے جائیں گے

نوٹ: یونین کونسل میں کمپیوٹرائزڈ پیدائش سرٹیفکیٹ کی سہولت نہ ہونے پر یونین کونسل کا مینوئل پیدائش سرٹیفکیٹ یا سکول سرٹیفکیٹ قابل قبول ہو گا۔

How to make 'B' form or ID card for children below 18 years of age
• One of the parents or a court-appointed guardian (guardian) must come along with the child's computerized birth certificate.
• If both parents are present, one will be the applicant and the other will be the attestator
• If one of the parents is present, the application form must be attested by the Gazetted Officer or public representative (MNA, MPA or local body officials and members).
• Presence of child is required for ID card. Child's photograph and fingerprints of children above 10 years of age will also be taken
Note: Manual Birth Certificate or School Certificate of Union Council will be acceptable if computerized birth certificate facility is not available in Union Council.
